We are committed to making the process of handling your insurance and financial needs as smooth as possible. As a new patient, you will meet with a Patient Accounts Representative to review your completed patient information form and to verify your information. Please bring your insurance identification card(s) with you so that we may place a photocopy in your file. If you are covered by more than one insurance company, please verify and inform us which company is the primary carrier. If there is every any change in your personal or insurance information, please contact our office promptly to update us.
Our practice participates with many insurance companies and will, in most cases, file claims to your carrier on your behalf. We also accept Medicare assignment. If you belong to an HMO or PPO plan, please verify with our office that our physician is in your plan prior to your first visit.
Some insurance plans require you to use a specific laboratory or to obtain referrals or preauthorization. Please verify these requirements with your carrier prior to your visit, as we wish to avoid any unnecessary expenses to you. If you need assistance with your insurance questions, our staff will make every effort to help you.
Co-payments are payable at the time of your visit, and we accept Visa and MasterCard for your convenience. We are also pleased to offer a monthly payment plan through Care Credit.
